Our team will help you understand and navigate your NDIS plan, providing one-on-one support as you work towards your goals. We work alongside you to coordinate your supports, ensuring that they reflect your individual needs, preferences and goals.
We collaborate closely with your family, carers and other members of your support network, helping you better manage day-to-day living. Our team will connect you with the right therapies, care, and community resources to help you build your capacity and promote choice and control, empowering you to achieve your goals.
Psychosocial Support
Pakari provides one on one support to assist you on your mental health journey. Our services are designed to assist you with daily living, health, employment and social connections both in the home and in the community.
Our team will work with you to develop strategies to build the skills you need to foster resilience, to recognise your strengths and achieve your goals. Pakari’s supports will empower you to actively engage in the community, manage everyday tasks and strengthen relationships with family and friends.
For NDIS participants, Pakari provides psychosocial supports working with your wider care team, aimed at improving functional ability and enhancing social and economic participation. Some activities our team can help you with:
